    Summary The Phoenix VA Health Care System Primary Care Physician Assistant position is located at the Main VA Clinic, Phoenix, AZ. Will work over night shifts. Document all aspects of patient care in a timely and accurate manner. Team member of the Advanced Practice Provider group. Assist in the operating room when appropriate. Provide day and night coverage of surgery and ICU inpatients as well as ED consults and outpatient clinic coverage, as needed. Select and interpret the results of diagnostic studies and laboratory studies commonly utilized in the general surgery setting. This includes, but is not limited to CT scan, ultrasound, MRI, PET scan, GI studies, and pathology reports. Maintain an updated understanding of surgical oncology treatment and follow up guidelines (such as National Cancer Care Network - NCCN) in order to provide evidence-based, timely care and follow up for cancer patients treated by the general surgery service Ability to be self-directed in clinical practice and in the management of patients with surgical needs across the continuum of care. Collaborate with attending surgeon, residents and expert consultants and demonstrates clinical competence in delivering quality care for surgical patients with complex medical and surgical problems.     Vighter is recruiting healthcare professionals (HCPs) to conduct basic screening assessments and provide healthcare services to individuals detained by Customs and Border Protection (CBP).     Summary:

    Vighter is recruiting healthcare professionals (HCPs) to conduct basic screening assessments and provide healthcare services to individuals detained by Customs and Border Protection (CBP). HCPs work administratively under the Regional Program Manager and Regional Deputy Program Manager while performing healthcare services under the medical direction of the National Medical Director and Physician Supervisors who collaborate with CBP’s Office of the Chief Medical Officer (OCMO). Healthcare is delivered in accordance with approved medical protocols and in compliance with Department of Homeland Security (DHS) directives.

    Vighter’s HCPs work alongside uniformed CBP Agents/Officers at numerous locations near the border in Texas, New Mexico, Arizona, California, and Florida. The medical staff must be capable of providing health care services for persons of all ages, to include infants, children, adults, and pregnant females. Candidates must be comfortable working in a detention setting. Services may be provided in a field setting with minimal facilities, a soft-sided (tent-type) facility, or a secure detention facility, such as a US Border Patrol Station. This is an amazing opportunity to work with diverse populations alongside a supportive team of healthcare professionals and administrative staff.

    Education, Licensure/Certification & Experience:

    + Must have a minimum of one year of experience in direct patient care and licensed/certified as a Physician Assistant (PA). Qualifying experience includes detention or correctional healthcare, residential healthcare, a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided direct patient care.

    + Successful completion of a US-based accredited Physician Assistant training program leading to licensure/certification as a Physician Assistant (PA). Must be accredited by the Accreditation Review Commission on Education for Physician Assistant (ARC-PA) or one of its predecessors. The degree issuing institution and/or program must be accredited by an entity recognized by the U.S. Department of Education.

    + Must have a current, active, valid, unencumbered license/registration/certification to practice healthcare in the state in which the candidate will work. License/registration/certification must not have been withheld, suspended, revoked, denied, surrendered, or lapsed due to an investigation.

    + Board Certification as a Physician Assistant by the American Board of Medical Specialties (ABMS) or National Commission on Certification of Physician Assistants (NCCPA) is preferred. Certification must be maintained if required as part of licensure for the jurisdiction’s licensing authority.

    + Must possess a current valid Basic Life Support (BLS) certification from one of the following approved organizations: American Heart Association (AHA) BLS for health care providers, AHA Advanced Cardiac Life Support (ACLS) certification, American Red Cross CPB/BLS for the professional rescuer, American Safety and Health Institute (ASHI) CPR and AED for healthcare providers and professional responders.

    + Application of medical principles, clinical procedures, and techniques.

    + Alertness and skill in providing care and reacting to emergency situations.

    + Basic safety, infection control, quality assurance, records management, statistical information gathering, etc.

    + Experience in a detention/correctional and/or pediatric setting is highly preferred

    + Bilingual in English and Spanish is highly preferred, but not required.

    Knowledge, Skills, & Abilities:

    + Provides direct patient care within their scope of practice to persons in custody, including children, pregnant women, and adults, to include emergency and urgent care.

    + Consults with a Physician for complex medical, pediatric, or psychiatric patients, completes orders recommended/authorized by the physician.

    + Performs history and physical assessment, behavioral health care, minor outpatient procedures, patient education, and refers any care beyond the scope of the medical unit to local medical facilities emergency departments or urgent care centers.

    + Performs vital signs electronically and manually, as indicated.

    + Ability to apply professional principles, procedures, and techniques for Physician’s Assistants to patient care.

    + Knowledge of drugs and therapeutics when prescribing and dispensing medication.

    + Maintains compliance with DHS and CBP policies and any overriding accreditation standards that may apply.

    + Ability to properly use equipment and supplies in simple diagnostic and treatment procedures.

    + Works within their scope of practice, facility Standing Orders/Standing Operating Procedures and protocols, and within the Collaborative Agreement.

    + Maintains patient confidentiality and privacy of medical records, complies with HIPAA regulations as applicable, performs record keeping functions in accordance with program policies and the system of records management.

    + Maintains non-involvement in the security aspect of patient care and custody.

    + Assures a safe and clean working environment and assists in the disposal of biologically hazardous waste on a regular basis, adheres to universal precautions, and takes appropriate infection control precautions.

    + Completes all annual trainings as required, or any additional training as assigned.

    + May be required to travel short notice.

    + May be required to serve at various medical units within the sector/region.

    + Duties will be performed on-site at medical units, flexible scheduling and overtime are optional, as available.

    + All medical documentation will be signed and dated, whether electronic or paper.

    + Participates with the Peer Review process, performs chart reviews as assigned, and assists with orienting new PAs or nurse practitioners, as assigned.

    + Provides clinical oversight of support staff as needed, to include nurses, certified nursing assistants, certified medical assistants, emergency medical technicians, and paramedics.

    + Communicates an end of shift report to the oncoming shift.

    + Participates in Medical Quality Management/Quality Assurance/Risk Management activities and initiatives as needed.

    + Provides emergency care to employees and visitors for life, death, and loss of sight/limb situations until Emergency Medical Services arrives.

    + Performs other duties and responsibilities as assigned.

    Duty Hours / On-call Requirements:

    + The CBP mission runs 24/7/365 to protect our country and as such shift schedules for HCPs include: day, night, weekday, weekend, and holidays.

    + We will work to accommodate desired shifts with locally hired employees, but there may be times where coverage is needed to meet contractual requirements.

    Travel:

    + This is not a travel position so candidates can be expected to work at alternate worksites within 50 miles of their permanent residence.

    + Note: Per the Joint Travel Regulations, candidates with a permanent residence less than 50 miles from a worksite are considered local hires and are not eligible to receive per diem or travel pay.
