**Parent Educator - Community Health Work Outreach**

**Northridge Hospital Medical** **Center** is seeking a passionate and dedicated **Parent Educator** to join our **Parents as Teachers (PAT) program** . We are committed to providing high-quality, home-based services that support parents in nurturing their children's healthy development from birth to kindergarten entry. This is a rewarding opportunity to make a significant difference in the lives of families within our community. This is a grant-funded position.
**Responsibilities**
+ Conduct home visits with families, providing individualized support and guidance based on their needs and goals.
+ Assess children's development using standardized tools and observations.
+ Develop and implement individualized family support plans that address developmental milestones, parenting skills, and family well-being.
+ Provide education and resources on a variety of topics, including child development, health, nutrition, safety, and parenting strategies.
+ Facilitate group activities and workshops for parents.
+ Maintain accurate and timely documentation of all client interactions and program activities.
+ Collaborate with other community agencies and professionals to ensure comprehensive support for families.
+ Participate in ongoing professional development opportunities.
+ Maintain confidentiality and adhere to all program policies and procedures.
**Qualifications**
+ High school diploma or GED
+ Two years experience working with children and families from diverse cultural and ethnic backgrounds.
+ Successful experience working with families, children, and/or parents, and knowledge of early childhood education best practices.
+ Knowledge of community-based service programs.
+ Home visiting experience preferred.
+ Experienced in Google Workspace
+ Bilingual in English/Spanish preferred.
+ AHA BLS is required within 90 days of hire.
+ LA County Fire Card upon hire within 90 days of hire.
+ Valid CA Driver’s License acceptable driving record and valid car insurance
**Pay Range**
$28.41 - $41.19 /hour
